- Certificates have changed following the digitization of clerical work related to Family Registers (戸籍//koseki) on June 4, 2011; entries removed from Family Registers before digitization due to marriages, deaths (etc) are not recorded on computerized versions. If you require these entries, please apply for a Heisei Invalidated Family Register (平成改製原戸籍//Heisei kaisei genkoseki) (¥750/certificate)
For further information, please see Digitization of Clerical Work Related to Family Registers (Japanese) - Information regarding addresses before June 2011 were recorded on Heisei Invalidated Family Register Attachments (平成改製原戸籍の附票//Heisei kaiseigen koseki no fuhyō) which were in use before digitization. If you need a full address history, a single ¥300 fee may not cover the full handling charge. For further details, please contact the appropriate office listed in Where to Apply below.
- Applications for Certificates of Entries Recorded on Notifications are only accepted in cases of special circumstances.
- Applications for Resident Records that contain Individual Numbers and/or Resident Record Codes are not accepted from anyone other than the person in question (herein referred to as the applicant) or someone from their household. If an application is made via proxy (someone entrusted to act on the behalf the applicant) documents will be sent to the address of the applicant (i.e. not the proxy). Please be aware that we cannot issue such documents directly to a proxy (this excludes applications made by legal representatives of those younger than 15 years old and guardians of adults).

- When applying in person (either the applicant themselves or a proxy) be sure to bring documentation that can be used to verify your identity (e.g., licenses, certificate of credentials, or other form of ID issued by a public body or private enterprise). Please note that this form of ID must display your name.

Please note that applications can be made by third parties where it is deemed that the intended use is legitimate. When the reason for an application is unclear, third parties may be asked to provide necessary explanation and additional documents. - When applying for documents via proxy (someone entrusted to act on the behalf the applicant), we require a Letter of Proxy (a document that has been signed by a mandator, or on which a mandator’s name is written and their personal seal is stamped).
Please note that a Letter of Proxy is not required when applying via proxy in the following cases:
- Documents no. 1-5: If the proxy is registered on the same Family Register or is directly related to the applicant (i.e., is the applicant’s parent, grandparent, child, grandchild, etc)
- Documents no. 8-9: If the proxy shares the same household as the applicant
- When applying in person (either the applicant themselves or a proxy) be sure to bring documentation that can be used to verify your identity (e.g., licenses, certificate of credentials, or other form of ID issued by a public body or private enterprise). When applying for a Personal Seal Registration Certificate, you will need to bring your Personal Seal Registration Card and an application form complete with your name, address, date of birth, etc.
A Letter of Proxy is not required when applying via proxy; however, the proxy must present the applicant’s Personal Seal Registration Card.Notes
- If you forget your Personal Seal Registration Card (except if your card is lost):
Registered applicants can still be issued a Personal Seal Registration Certificate if they present a form of photo ID issued by a public body (e.g., your Individual Number Card (My Number Card), driver’s license, etc) only when applying in person. For further information, please contact your local ward office Citizens Affairs Division or branch office. - If you have lost your Personal Seal Registration Card:
Please submit a Notification of Lost Personal Seal Registration Card (印鑑登録証亡失届//Inkan tōrokushō bōshitsu todoke). Your personal seal registration will be terminated, and you will be required to register once more.
