Within 7 days of the day you are informed of the death

In the following order of priority:

  1. Relative
  2. Person living with the deceased
  3. Landlord, landowner, or person responsible for managing the residence or land of the deceased
  • Death Notification form *Note 1
  • Death Certificate or Postmortem Certificate
  • Application for Cremation Permission *Note 2
  • Residence Card or Special Permanent Resident Certificate
  • National Health Insurance Card (If you have one) *Note 3
  • Atomic Bomb Survivor’s Certificate (if you have one) *Note 3

Citizens Affairs Division in the Ward Office of the area where the death took place or the applicant lives

  • *Note 1: Death Notification forms are available at the Citizens Affairs Division in Ward Offices and Branch Offices.
  • *Note 2: Please file an Application for Cremation Permission, and submit a Death Notification.
  • *Note 3: If the deceased was enrolled in National Health Insurance or possessed an Atomic Bomb Survivor’s Certificate, a funeral allowance is paid. The procedures for receiving the allowance can only be taken on days when municipal offices are open.
