1. Benefits of the Individual Number Card

The Individual Number system was introduced in October of 2015, and applications for Individual Number Cards were accepted starting in January of the following year. Get an Individual Number Card to enjoy the following benefits for its usage.

In cases where you must show your Individual Number, such as procedures of year-end adjustments or applications for child allowances, you can use the card as a document for verifying your Individual Number.

Since it can be used as a document to verify your identity for public agencies, the Individual Number Card is the only card that can be used to both verify your identity and indicate your Individual Number.

You can acquire copies of your residence certificate, seal registration certificate, and the like at convenience stores.

You can use e-Tax. There are plans to use the Individual Number portal for online applications such as procedures related to child rearing.

2. Application Method for the Individual Number Card

You can apply for an Individual Number Card using one of the following methods.

Application by mail
Application by smartphone
Application by PC

Related Information

If you do not have a personal computer at home, you can make an online application for the Individual Card by taking your photo using one of the terminals for the Personal Information Cooperation Record Disclosure System (Mynaportal) installed in the Citizens Affairs Division of each ward office.
Who can make an application:
The applicant him/herself (proxy application is not allowed.)
Where to apply:

Citizens Affairs Division of the ward office of residence (You cannot make an application at a Branch Office or Liaison Office.)
Note: The application procedure takes time, so please come to the office early.

Necessary items for application:
Bring the completed Application Form for Issuance of the Individual Number Card[PDFファイル／237KB], which has been mailed to you together with the Notification Card. If you have lost the application form, contact the Citizens Affairs Division of your local ward office.
If you wish to receive your Individual Number Card by certified mail with delivery restricted to the addressee:
If you bring all of the following documents 1–3 in addition to the Application Form for Issuance of the Individual Number Card, you can receive your Individual Number Card from your ward office/branch office by certified mail with delivery restricted to the addressee, about one month after application.

Notification Card (to be returned to the municipality)
Basic Resident Registration Card (only for those who have one; to be returned to the municipality)
Identity verification documents; your identity is verified by either of the following methods, a or b:
1. One item from A
2. If you do not have any of the documents listed in A, two items from B

Note: For both a and b, we may ask you some questions to verify your identity, if necessary.

List of identity verification documents

A	Identification documents with face photo, issued by government/public agencies (must be within the valid period): Basic Resident Registration Card (with photo), driver’s license, resident card, special permanent resident certificate, passport, certificate of driving history (issuance date must be on or after April 1, 2012.), physical disability handbook, health and welfare handbook for the mentally disabled, rehabilitation handbook, landing permit for temporary refuge, permit for provisional stay
B	Permits or qualification certificates with face photo attached: Seamen’s competency certificate, small boat operator’s license, electrician’s license, radio operator’s license, train driver’s license, passing certificate for aircraft dispatcher’s competence examination, permit for possession of hunting guns and air guns, certificate of qualified electrical engineering specialist, certificate of certified electrical engineer, certificate of airworthiness inspector, airmen competence certificate, certificate of building lots and buildings transaction manager, official identification document issued by government/public agency, etc.
B	Documents containing either the full name and date of birth, or the full name and address, issued by government/public agencies or corporations: Seaman’s pocket ledger, war disabled notebook, certificate for receiving firearms training, pass certificate, welfare benefit recipient certificate, health insurance certificate or long-term care insurance certificate, medical benefit certificate, pension certificate, certificate of child-rearing allowance, certificate of special child-rearing allowance, bankbook (indicating address), employee identification card, student identification card, various documents in which the school name is stated, etc.

You will need to set your personal identification number. Please determine your personal identification number before you visit the service counter.

Personal Identification Number (PIN) for electronic certificate for the bearer’s signature	Electronic certificate for the bearer’s signature: Used when filing electronic applications such as e-Tax; validates that the electronic document submitted over the Internet is genuine and has not been falsified.	Six to a maximum of sixteen alphanumeric characters
PIN for electronic certificate for user identification	Electronic certificate for user identification: Used for convenience store issuance services and when logging in to the Disclosure System of Personal Information Cooperation Record, etc.; validates the identity of the person logging in as yourself.	Four-digit numbers You can set the same personal identification number for these three cases.
PIN for Basic Resident Registration Card	PIN necessary for clerical work related to the Basic Resident Registration Card
PIN to support entering items on the card	PIN needed to read information (individual number, name, etc.) from an IC chip, in e-applications, etc.

Please note that if you cannot prepare some of the required documents, you will need to visit the Citizens Affairs Division of your ward office, or a branch office, to be issued your Individual Number Card.
