How to Receive Your Individual Number Card

If you receive the "Individual Number Card Issuance / Electronic Certificate Issuance Notification and Inquiry" (hereinafter refer to as "Issuance Notification (Postcard)"), please bring the following documents and come in person to receive your Individual Number Card.

* Persons under the age of 15 and adult wards must be accompanied by their legal representative (person in parental authority, guardian, etc.)

The legal representative should also not come alone, but must come together with the applicant.

Remove the blinder sticker on the Issuance Notification (Postcard) to check the issuing location.

The Issuance Notification is sent approximately one month after your application for issuance of the Individual Number Card.

1. Documents Required if Received in Person

* Persons under the age of 15 and adult wards must be accompanied by their legal representative (person in parental authority, guardian, etc.)

The legal representative should also not come alone, but must come together with the applicant.

  • Issuance Notification (Postcard)
  • Individual Number "Notification Card"
  • Basic Resident Registration Card (if you have one)
  • Documents for personal identification (Your identity will be confirmed using either of the following two methods.)

 (1) One point from A
 (2)If you do not have a document from , two points from B
* For both and ‚, we may ask you questions as necessary to confirm your identity.

Persons under the age of 15 and adult wards must be accompanied by their legal representative (person in parental authority, guardian, etc.) and also require the following documents in addition to the above.

  • Documents for personal identification of the legal representative (in the same way with the applicant in person, documents from pattern  or ‚ are required)
  • Documents for confirming the attorney's authority of representation (a copy of family register or other qualifying documents)
    * This is not necessary if the principal is under 15 years of age, but his/her registered domicile is in Hiroshima City or the principal and his/her legal representative are in the same household and are also parent and child.

List of Personal Identification Documents

A

Personal ID with a photograph issued by a government or public agency (still valid if there is an expiration date)

Basic resident registration card (with photograph), driver's license, residence card, special permanent resident certificate, passport, certificate of driving history (issued on or after April 1, 2012), physical disability certificate, mental disability certificate, rehabilitation certificate, permit for temporary refuge, permit for provisional stay

B

Permit or qualification certificate with photograph

Seaman's certificate of competency, small craft operator certificate, electrical engineer's license, radiotelephone operator license, train driver's license, passing certificate of aircraft dispatcher competence examination, hunting gun/air gun permit, permit for possession of hunting guns and air guns, certificate of qualified electrical engineering specialist, certificate of certified electrical engineer, certificate of airworthiness inspector, airmen competence certificate, certificate of real estate broker, personal ID issued by government and public agencies for employees, etc.

Document issued by a government or public agency or corporation that lists your "name and date of birth" or "name and address"

Mariner's pocket ledger, certificate of sick and wounded retired soldiers, certificate of the qualification for receiving shooting training, examination certificate, certificate of public assistance recipient, insurance card for health insurance or nursing-care insurance, medical care certificate for the elderly, various pension certificates, child rearing allowance certificate, special child rearing allowance certificate, savings account passbook (limited to those that list your address), employee ID, student ID, documentation that lists your school name, etc.

* If your address or name has been changed due to moving or marriage, first have the agency that issued your identification documents replace your address or name before visiting for your Card.

* If the text or picture on your identification documents has become dirty or worn and cannot be confirmed, have the document reissued before visiting for your Card.

* Only identification documents that are valid at the time of your visit to the said Office will be accepted.

* We will take a copy of your identification document.

2. Documents Required if Received by the Representative

A representative can be appointed to receive the Card, only in cases where it is difficult for the applicant to come to the issuing location due to unavoidable circumstances, such as an illness or disability.

* Being busy at work or school is not considered unavoidable circumstances.

  • Issuance Notification (Postcard)
    * The Card applicant must fill in and affix their seal to the "Response" section and PIN section on the back side, and be sure to apply the blinder sticker to the PIN section.
  • Individual Number "Notification Card" of the applicant (himself/herself)
  • Basic Resident Registration Card (if you have one) of the applicant (himself/herself)
  • Identification documents for the Card applicant (himself/herself) (The applicant's identity will be confirmed using one of the following three methods.
     (1) Two points from A
     (2) One point from A and one point from B
     (3) Three points from B (with at least one point of those including a photograph)
  • Identification documents for the representative (The representative's identity will be confirmed using one of the following two methods.)
     (1) Two points from A
     (2)One point from A and one point from B
    * Identification documents for both the Card applicant and their representatives are required.
    * Both the applicant and representative must have at least one point of identification with a photograph issued by a government or public agency.
  • Document that verifies that it is difficult for the applicant to come to the Office (E.g., doctor's note, documents that verify the applicant has been admitted to a facility)
    * Being busy at work or school is not considered unavoidable circumstances.
    * This is required even if the legal representative is coming alone because the applicant is under 15 years of age or is an adult ward.
  • Documents for confirming the attorney's authority of representation
    For legal representatives: A copy of family register or other qualifying documents (*Not required for those with a registered domicile in Hiroshima City.)
    For other representatives: A letter of proxy (Fill in and affix seal to the "Letter of Proxy" section of the Issuance Notification (Postcard).)

3. PIN

The desk where you receive the documents will have you set at least two PINs. Please decide on numbers cannot be easily guessed in advance.

* If the appointed representative comes to receive the Card, the Card applicant himself or herself must fill in the PIN section on the Issuance Notification (Postcard) and be sure to apply the blinder section, then hand that to the representative. Office staff will input the PINs.

PIN for using signature electronic certificate

Signature electronic certificatežžžIf performing an electronic application, such as for e-Tax or the like, this function confirms that the electronic document sent over the Internet has not be altered.

Set this using 6 to 16 alphanumeric characters.

PIN for using electronic certificate for user authorization

Electronic certificate for user authorizationžžžThis system verifies that user identification when using the convenience store certificate issuance and when logging into your personal page.

It can be set with a 4-digit number.
All three can be set to the same PIN.

PIN for aiding input of items listed on certificate

PIN required for reading Individual Number or name information from the IC chip during electronic applications

4. Issuance Location

The Individual Number Card is issued at the Ward Office's Citizens Affairs Division or branch office with jurisdiction for the address on your residence certificate. (Cards are not issued at liaison offices.)

The Individual Number Card Issuance Desk tends to be busy on Mondays, from 10 AM till about 2 PM.

If the Individual Number Card Issuance Desk is busy and the waiting time is extremely long, or if same-day issuance is difficult due to a nation-wide system error, we may have you fill in your Individual Number Card PINs on a PIN setting request form, with our staff setting the PINs and sending your Individual Number Card in the mail at a later date.

When issuing an Individual Number Card, we go through personal identification documents one by one and perform PIN input setting procedures; it can therefore take about 20 minutes per person. It can also take even longer depending on the content of the procedure and how busy the Desk is. Please come when you have plenty of time.
