Birth Notification

Birth Certificate, personal seal, Maternal and Child Health Handbook

Submission of Birth Notification Card

Please submit a Birth Notification Card (postcard) which was delivered to you when you received the Maternal and Child Health Handbook.

If the birthweight of your baby is less than 2,500 g, you are required to notify the municipal office of this without fail.

Hello Baby Services

Commissioned welfare volunteers and childcare commissioners visit households who have a baby aged 4 months or younger.

The following procedures are required only for relevant persons. Relevant persons must take these necessary procedures without fail.

Purchase National Health Insurance

Health insurance card of the head of the household

When your baby is born in a household enrolled in National Health Insurance and is not covered by other health insurance plans, you are required to purchase National Health Insurance for the baby.

Health and Pension Division in your Ward Office*

 

Application for a lump-sum maternity allowance under National Health Insurance

(Those who do not use a direct payment system or a substitute payee system)

National Health Insurance card, document proving childbirth (Maternal and Child Health Handbook, etc.), item showing a savings account number in the name of the head of the household, personal seal, receipt/statement of expenses for childbirth, document issued by a hospital proving that you do not use a direct payment system (this is not necessary when the receipt/statement contains this information.)

(Those who use a direct payment system and still can receive the balance)

National Health Insurance card, document proving childbirth (Maternal and Child Health Handbook, etc.), item showing a savings account number in the name of the head of the household, personal seal, written document concerning a substitute agreement, receipt/statement of expenses for childbirth

When those enrolled in National Health Insurance give birth, they are eligible for this allowance, unless they receive such allowance from other health insurance plans.

There are three ways to receive the lump-sum maternity allowance: a direct payment system (you are required to sign a substitute agreement concerning reception of the allowance with a hospital), a substitute payee system (you are required to obtain the approval of a hospital and make a prior application), and making an application after childbirth to receive the allowance. When you use a direct payment system or a substitute payee system, you only have to pay the amount exceeding the lump-sum maternity allowance at a hospital.

Please note that the allowance will not be paid if two years have passed from the day following childbirth.

* During or after the 12th week of pregnancy, you can receive the allowance even if you have a miscarriage or stillbirth.

* For those who had been enrolled in health insurance of a company where you worked for more than a year (excluding the National Health Insurance Association) and gave birth within six months after retirement, the lump-sum maternity allowance is paid by the health insurance company of the company. Please contact any relevant person of the health insurance company.

Application for certification of child allowance

Personal seal, deposit passbook in the name of the applicant, health insurance card of the applicant, etc.

Please apply for certification of child allowance within 15 days from the day following childbirth. If your application is made later, you cannot receive the allowance retroactively.

Child Allowance Certificate, copy of your family register, copy of your Resident Record, personal seal, etc.

Notification of a change in household members in a municipal house

When the number of people living together in a municipal house increases due to the birth of a child, you are required to take the prescribed procedures. For more information, please contact the Building Division.

Notification of a change in the number of toilet users

When the number of household members increases due to the birth of a child, please notify the Environmental Business Division, Hiroshima Urban Development Corporation.
