* Note 1: The death registration form can be obtained at theCitizens Affairs Division of your ward office or branch office.
* Note 2: File the application for burial/cremation permittogether with the death registration form.
* Note 3: If the deceased was enrolled in national healthinsurance or possessed an A-bomb Health Book then the personsarranging the funeral are eligible to receive a funeralallowance.
