Type Registration period Registrant (applicant) Required items Place of registration
Death registration Within seven days of the day you are informed of the death
  1. Relative
  2. Cohabitant
  3. Landlord, land owner, or person responsible for managing the residence or land of the deceased
    (In that order)
Death registration form
*Note 1
Application for burial/cremation permit
*Note 2
Alien registration card
National health insurance certificate
(Only for persons enrolled in national health insurance)
*Note 3
A-bomb Health Book (Only for persons who possess the health book)
*Note 3
Municipality of either the place of death or the residence of the registrant

* Note 1: The death registration form can be obtained at theCitizens Affairs Division of your ward office or branch office.

* Note 2: File the application for burial/cremation permittogether with the death registration form.

* Note 3: If the deceased was enrolled in national healthinsurance or possessed an A-bomb Health Book then the personsarranging the funeral are eligible to receive a funeralallowance.
