Necessary Items

  • Completed Application Form
    Postal application forms can be downloaded below.
    Please note that applications via post can be made in any format, not only by application form, as long as you include the following information:
    1. Your current address, your name (with furigana, if necessary), and your date of birth.
      Those who currently do not live in Hiroshima City should also include their address from when they did.
    2. What you intend on using the certificate for.
    3. The type and fiscal year of the certificate you need (if you are applying for a Certificate of Income, the certificate will contain information regarding your income from the previous to the given fiscal year).
    4. How many copies you are applying for.
    5. Daytime contact information (used to contact you if there is any information missing from your application, etc).
  • Self-Addressed Stamped Return Envelope
  • Postal Money Order for Administration Fees
    Please buy postal money orders at the post office to ensure you are paying the correct amount.

Where to Send Your Application

Send applications for Certificates of Municipal and Prefectural Residents Taxes Ledger and Certificates of Tax Payment (for Municipal and Prefectural Residents Taxes) to the Municipal Tax Office Management Section or Tax Room in charge of your local ward.
