COVID 19: Vaccination Information (as of Nov. 29, 2023)

Who can receive the vaccine?

Those aged six months or older and have a resident record in Hiroshima City are eligible to receive the vaccine. However, the type of vaccine you can receive differs depending on age. Please see the following for details:

Ages 12 and over (Japanese)
Ages 5-11 (Japanese)
Ages 6 months to 4 years (Japanese)

Vaccines will be available until March 31, 2024 and are free.

How to get your vaccine

Receive your vaccination coupon

You will need your vaccination coupon when making your appointment. Coupons will be sent to the address on your Resident Record. If you still have any unused vaccination coupons, you will not be sent new ones.

Make an appointment for your vaccine

Individual vaccinations at hospitals, clinics, etc.

What to bring to your appointment

For your first and second dose

  • Your vaccination coupon
  • Your completed Pre-vaccination Questionnaire*
  • Form of ID (passport, Resident Card, health insurance card, driver’s license, etc)

For boosters

  • Vaccination coupon and pre-vaccination questionnaire sheet (they will be on the same slip; fill out the questionnaire in advance and do not tear the two apart).
  • Form of ID (passport, Resident Card, health insurance card, driver’s license, etc)

*You will need to fill out the questionnaire in Japanese. If you cannot read/write in Japanese, get a friend to help you. You can find translated versions of this questionnaire on the Ministry of Health, Labor, and Welfare website<外部リンク>.

Note: if your vaccination coupon does not arrive within the appropriate timeframe, or if you have lost your vaccination coupon, you will have to apply for a new one. For more information, see here (Japanese).

Vaccine Passports

​After you receive your COVID-19 vaccination, you will be issued a Certificate of Vaccination for COVID-19 that lists the date, place, and type of vaccine you received. As this certificate allows you to know when and which type of vaccine you received at a glance, it is very important that you keep this certificate in a safe place.

You can also apply to the City of Hiroshima for a Vaccine Passport that certifies you have been vaccinated for COVID-19, free of charge.

Digital Vaccine Passport via App

You can receive a digital vaccine passport via the official Certificate of Full COVID-19 Vaccination app (released by the Japanese government). To receive this digital certificate, you will need a smartphone and your Individual Number Card (My Number Card).

You can still receive a paper copy of your vaccine passport. Please send your application form to the City of Hiroshima, preferably via postal mail.

Necessary documents for Paper Vaccine Passport

  • Vaccine Passport Application Form (English and Japanese)
  • Copy of your travel passport (for those who need the Vaccine Passport for travel purposes)
  • ID (passport, Resident Card, insurance card, driver's license, etc., for those using the passport for domestic purposes)
  • One of the following documents (copy acceptable) to confirm records by the City of Hiroshima: Vaccination Coupon, My Number Card, ID (complete with current address)
  • One of the following documents (copy acceptable) to confirm that you have been vaccinated: Certificate of Vaccination for COVID-19, record of vaccination, patient copy of your medical questionnaire
  • Return envelope (please fill out your name and address and affix an 84-yen stamp)
  • If your travel passport includes your maiden name, different last name, or different first name, please include documentation which can verify this information (copy acceptable).
  • If applying via proxy, you will need to include a letter of proxy and ID for your proxy (copy acceptable)

Note: Please write “Application for COVID-19 Vaccine Passport Enclosed” in red letters on the envelope.

If you would prefer to deliver your application in person, please bring it to either of the following locations:

  • Health Promotion Division Temporary Office
    16F Hiroshima City Hall
  • Mutual Community Support Division of your local ward office

How to receive your paper vaccine passport

Vaccine passports will be issued 5-7 days after the City of Hiroshima receives your application. Please note that it may take more time to issue your passport depending on circumstances such as processing a large number of applications, mistakes on the application form, national holidays, etc.

When your passport is ready, it will be mailed to your home address. If you would like to receive your passport in person, please let a member of staff know when you drop off your application.

Other information

  • Only those who have a registered address in Hiroshima City or have received their vaccine using the vaccination coupons sent by the City of Hiroshima can apply for a vaccine passport from the City of Hiroshima.
  • Those who moved into Hiroshima City after receiving their vaccine elsewhere should apply for a vaccine passport at their previous residence.
  • Those who moved after their first dose of the vaccine and received their second dose elsewhere will need to apply at each separate municipality.
