After you receive your COVID-19 vaccination, you will be issued a Certificate of Vaccination for COVID-19 that lists the date, place, and type of vaccine you received. As this certificate allows you to know when and which type of vaccine you received at a glance, it is very important that you keep this certificate in a safe place (copies or photos of this certificate are also considered valid).
Note: The Certificate of Vaccination for COVID-19 is attached to the righthand side of the Vaccination Coupon that you must bring when going to receive your vaccination (see below for reference; vaccination coupon is outlined in green, certificate is outlined in red).
Certificates of Full COVID-19 Vaccination (hereafter “vaccine passport”) will be issued to those who have received two doses (or more) of the COVID-19 vaccine.
Beginning December 20, 2021, you can now receive a digital vaccine passport via the official Certificate of Full COVID-19 Vaccination app (released by the Japanese government). To receive this digital certificate, you will need a smartphone and your Individual Number Card (My Number Card). If you do not have a smartphone or Individual Number Card, you can still receive a paper copy of your vaccine passport.
In addition, vaccine passports, which were previously only issued for the purpose of travel overseas, can now be issued for purposes other than travel overseas beginning on December 20 (e.g., for those who have lost their Certificate of Vaccination for COVID-19 received at the time of vaccination, etc).
Please note that depending on circumstances, such as registration status in the COVID-19 Vaccination Record System (VRS), you may not be able to receive your digital vaccine passport. In this case, please consider applying for a paper vaccine passport.

If you would prefer to deliver your application in person, please bring it to either of the following locations:
Vaccine passports will be issued 5-7 days after the City of Hiroshima receives your application. Please note that it may take more time to issue your passport depending on circumstances such as processing a large number of applications, mistakes on the application form, national holidays, etc.
When your passport is ready, it will be mailed to your home address. If you would like to receive your passport in person, please let a member of staff know when you drop off your application.
