Certificate of Full COVID-19 Vaccination

Certificate of Vaccination

Those who have completed their COVID-19 vaccination can prove their status via the following:

  1. For medical workers: Record of Vaccination for COVID-19
  2. For non-medical workers: Temporary Certificate of Vaccination for COVID-19 (attached to your Vaccination Coupon)

However, medical workers who need a vaccination certificate issued by the City of Hiroshima may apply for one. Non-medical workers who have lost their Temporary Certificate of Vaccination for COVID-19 may also apply for a vaccination certificate. In both cases, only those with Resident Records in Hiroshima City are eligible to apply.

Application

Necessary Paperwork

For medical workers:

  • Application for Vaccination Certificate
  • Copy of Record of Vaccination for COVID-19
  • Copy of ID (driver’s license, health insurance card, My Number card, etc) (only if you have lost your Record of Vaccination for COVID-19)

For non-medical workers:

  • Application for Vaccination Certificate
  • Copy of ID (driver’s license, health insurance card, My Number card, etc)

Medical workers and non-medical workers will need to fill out different portions of the application. Please see the following for examples of each:

Submitting Your Application

Via Postal Mail

Please send your application to the following address:

Health Promotion Division, Health Department, Health and Welfare Bureau, City of Hiroshima
1-6-34 Kokutaiji-machi, Naka-ku, Hiroshima 730-8586

  • Note 1: Please write the following phrase on the envelope in red letters:

    新型コロナウイルスワクチン接種済証明願 在中

    (Application for Vaccination Certificate Enclosed)

  • Note 2: Please include a self-addressed, stamped return envelope.

Submitting In Person

Please bring your completed application form to the Health Promotion Division on the 13th floor of Hiroshima City Hall.

Notes

1. As we will need to confirm the record of your vaccination to issue your certificate, it may take 2-3 months depending on circumstances. *

*If medical workers do not have a Record of Vaccination for COVID-19 or if it is necessary for the medical institution at which a non-medical worker received their vaccination to put information into the vaccination record system.

2. There are no fees to issue this certificate.

3. For those who would like to have their certificate mailed to them, please include a self-addressed, stamped envelope when applying.

4. It will be necessary for us to contact you should anything be missing or incomplete on your application. Be sure that you include your contact information (a phone number that you usually use) whether applying via post or in person.
